영어로 쓰는 차팅에 대해서 얘기를 나눠 볼까 하고 이렇게 글을 들고 왔습니다
한국에서 일 할 때는 정말 차팅
이런 말하기 쫌 그렇지만 기깔 나게 잘했다고 생각합니다 ㅋㅋㅋㅋㅋㅋㅋㅋㅋㅋㅋㅋㅋㅋㅋ
내가 한 것, 수행한것, 듣고, 보고, 전달 받은 것
바로바로 기록에 남겼고
신규때 , 제가 남긴 기록 덕분에
사건에 휘말리지(?) 않았던 기억 때문인지
다른건 몰라도 기록은, 꼭 자세히 남겨야 겠다고 생각했습니다
그와 같은 마음가짐으로 미국에서 넘어왔는데
이게 왠걸,, 키보드에 손을 가져다 댔는데 진짜 뭐라고 써야할지 모르겠는거예요
한국말로는 쓸 내용들이 머릿속에 순서대로 있는데
영어로는,,이걸 일기장 처럼 쓰자고 하니
I checked vital signs and all stable... 뭐 이렇게 쓰고는 손을 움직일 수도 없었죠
노티한걸 적을려고 해도 와,, 울고 싶었어요 ㅋㅋ ㅠㅠ
이걸 어떻게 해야할지
단어 공부도 안한 과거의 나를 뚜들겨 패면서,,
환자랑 대화 하는 것도 미쳐버리겠는데
그래서 프리셉터랑 동료들 하는 말들
차팅에 적혀있는 단어들, 문구들, 말들을 참고 삼아
다 제 차팅에도 적어보려고 노력했어요
당연히 완벽할 수는 없겠지만, 그래도 유치원생 일기장 정도야 벗어날 수 있지 않을까 싶어서 ㅋㅋㅋㅋㅋ
그래서 제가 공부했던 자료들을 공유해보려고 합니다
간호차팅을 어떻게 하는지, 어떤 단어들을 자주쓰는지 같이 알아보면 좋을 것 같아요
미국에서는 EPIC이라는 EMR 프로그램을 썼구요
저는 Med/Surg/Tele unit 에서 일을 했습니다. Assessment 하는 칸에 굉장히 자세하게 기록 할 수 있기 때문에, 본인의 스타일에 따라 자세히 기재하지 않는 경우도 있는데요. 한국도 생각해보면 마찬가지였던것 같아요.
각자만의 스타일이 다양하게 있고, 정답은 절대 없고. 문법 보라고 올려둔 것 절대 아닙니다!
어떤 단어들을 자주 쓰고, 내가 써먹을 수 있는 표현이 있다면 같이 공부하려고 정리해봅니다!
Received pt at 7:30 am from night nurse. Unable to assess orientation. Pt in bed, non-verbal and in no apparent distress. Pt was admitted with the diagnosis of respiratory distress. Pt is no longer on continues telemetry monitoring. Vitals stable as documented in chart. Maintained on FiO2 30%, patient tolerating well with O2 saturation at >99%. Peg in place, peg site intact, feeding held as ordered by MD. Peg dressing is clean and intact. Patient maintained on continues pulse oximetry monitoring. suctioning rendered orally and via trach with clear to minimal white secretions. Aspiration precaution is observed and patient will be continuously monitored for airway patency throughout the shift. Skin and incontinence care rendered. Safety precautions in place. Bed alarm on at all times. Hourly purposeful rounding in progress. Will continue to monitor closely

(여긴 없지만) PICC- '픽라인'
Bed alarm on, Call bell within reach, Hourly rounding in progress 는 늘 꼭 넣으라고 했던 프리셉터가 기억이나네요 ㅋㅋㅋㅋ
Received pt at 7:30am from night nurse. Pt is in bed, non -verbal and in no apparent distress. Patient opens eye spontaneously when spoken to, and also to tactile stimuli. Pt was admitted with the diagnosis of respiratory distress. Vitals stable as documented in chart. HR displaying 90’s to 150’s, sustaining in the 120’s. Dr Fharent notified and is aware. REceived with vent PRVC mode, tidal volume 450, RR 10 Peep 5, Fio2 30%, Patient tolerating well with O2 saturation. Ped in place and intact. Peg feedings promote at 70ml/hr ongoing as ordered. Patient tolerating well. Bed alarm on at all times. Hourly purposeful rounding in progress. will continue to monitor closely throughout shift
같은 환자가 Tele monitoring 을 했을 때 차팅을 가정했어요
닥터 이름도 다 허구 입니다
HR 기록 할 때 Display, sustain, showing symptoms 이런 단어들도 쓸 수 있고
원활히 feeding 이 진행될 때 tolerating well 이렇게도 쓰는것 같네요
여전히 빠지지 않는 Bed alarm, rounding, will monitor closely ㅋㅋㅋㅋㅋㅋㅋㅋㅋㅋㅋㅋㅋㅋㅋㅋㅋㅋ
Patient noticed to have an episode of V-tach 20 beats, nonsustained on the monitor Pt denies any chest pain, palpitations, dizziness, headache at the moment. Vitals done and is stable. MD notified and came to assess pt at bedside. EKG done
Patient noticed again to be in sustained V-tach. Denies chest pain, SOB, palpitation, dizziness, HA. VS done and is stable. MD notified and came to assess pt at the bedside. EKG done. Metoprolol succinate XL 50mg tablet ordered and is given. Pt’s HR was elevated and peaked at 160. MD made aware. As per MD and cardiologist pt was monitored and no interventions were performed.
몇비트만 지나가고, 다시 원래의 리듬대로 돌아오는
nonsustained V-tach 도 차팅에 자주 썼던것 같아요. 환자들 자는사이에도 지나가고 그래서
"Tele monitor displayed 5 beats nonsustained V-tach. Pt denied any chest pain, dizziness and SOB. V/S done. Dr Amy notified. As per MD order, monitoring will continue. No order made at the moment"
이렇게? 남기기도 해요
시험도 그렇고, 병원에서도 왠만하면 정말 다들 아는 약자 말고는 막 줄여쓰지 말라고 하는데, 차팅에 보면
" VSS WNL "
이라고 적어놨길래,,얜 뭐지? 하고 봤더니
" Vitals stable within normal limit "
제 눈엔 어색하니, 모르겠더라구요...
Heparin drip started at 16:24 ; infusing at 7.1ml/hr. pt tolerating well; bleeding precautions observed. Endorsed to night nurse for continuity of care
CVA 환자들도 Tele를 하러 오기 때문에, CHF, CVA 환자가 병동 메인 환자들이였어요
헤파린 드립 시작이나, 용량 변경 사항 있으면 남겨주고. 시시각각 변하는 건 Medication 창에서도 확인이 가능했기 때문에
시작이나, 엔딩 혹은 다른 처치나 약으로 바뀌는 경우에 차팅남겼어요
단어들이나 말들은
병원이 아니고 뭐 파트타임이나, 스쿨널스나 어떤 상황이던
간호기록을 남겨야 할 때 쓸 수 있으니
한번 봐두면 우려먹을 수 있는것 같아요 😏
분명 어딘가에 더 공부했던게 있는것 같은데
